Reservation Request Form - Neighbor Island Shows

Please fill out a separate form for each grade or grade grouping. For example, if you want to request performances for a grade K-3 group and for a grade 4-6 group, please fill out separate requests. If you want to attend multiple shows with the same group, you can fill out a single form and select multiple shows.
Please note: This form is for Neighbor Island Performances only.

For reservations for performances at Tenney Theatre on O‘ahu, please use Tenney Reservation Form.

For reservations for performances that tour to your school (preschool/kindergarten), please use Touring Shows Reservation Form.
