APPLICATION FOR SCHOOL ACCREDITATION

ON-THE-JOB TRAINING
Before accomplishing this form, make sure you have read the guidelines for School Accreditation.

If your school is already accredited, please proceed to the Student on-the-job training application.

Interested schools shall prepare an electronic copy of the following documents (in pdf format):

1. Cover Letter indicating the following:

• Request for Accreditation of School for MIAA OJT Program
• Courses to be accredited
• Number of students to be endorsed
• Required number of OJT hours
• Timeline and duration of OJT
• Full name, position and contact details of OJT Coordinator
• Name of Signatory in the Memorandum of Agreement

2. Accomplished Application Form

3. School Profile

4. Government Permit from the Commission on Higher Education (CHED)

Cover Letter must be signed by the Dean/Head of School addressed to:

If approved, a Memorandum of Agreement (MOA) shall be signed by and between Assistant General Manager for Finance and Administration and Dean/Head of the School.

NOTES:

1. Application for school accreditation shall be made by the school’s designated OJT coordinator, not by the students. Application with incomplete requirements will not be processed.

2. All documentary requirements should be certified true copies if the original copy of the documents cannot be submitted.

3. The accreditation shall commence upon approval of the MOA and shall remain valid until terminated by either party giving a notice of such termination to the other party.

IMPORTANT:

Please ensure that all uploaded documents are clear. The MIAA may require other related documents as it deems necessary.
