• Marketing Outreach Design Team Request Form

  • Please complete the form below to request services from the MOD team.

    To ensure your request gets processed, please be sure to do the following:

    1. If you are part of a Registered Club, please complete the Clubs Form before submitting the form.
    2. If you are a part of any Services and Activities (S&A) Funded Programs, and are requesting a Custom Design please complete the Campus Resource Form before submitting the form.
    3. Complete this MOD Request 4-5 weeks before your event date (Be sure to leave enough time for promotion before your event!). More time will be needed for large or complex projects and during busy times of the quarter. Projects will be approved and prioritized based on importance and relevance to the student body.

    Important Note: We cannot move forward with any requests until your corresponding form has been completed.
