    Facilites Request Form

  • The purpose of this Facilities Request Form is

    1) to recieve permission to use one of our Facilities

    2) submit your event or activity to be placed on the Calendar.

    In order to ensure that each ministry and event is able to make use of UCUMC's campus it's important that we steward well what we've been given.  For proper stewardship, permission to use UCUMC's facilities must be given by the Board of Stewards. This applies to all events and rehearsals.

    The following pages will take you through the details of your event.

    You must submit this form a minimum of two weeks prior to the proposed start date of your event.

  • You are required to clean up after you event. This includes cleanup of all materials used, cleaning surfaces, and taking out the garbage. You will also be responsible to set-up and reset-up the furniture for this event. Please plan ahead so that you have the neccessary number of volunteers to assist in this. Failure to comply could result in loss of security deposit.

    Sound, Lighting, & Projection

  • If you need the use of the Tech Arts equipment, which includes the use of the sound system, projection system and special lighting (where applicable) then you must fill out a separate Tech Arts Request Form. Be sure that you complete this Facilities Request Form.

  • Certificate of Liability or Hold Harmless Agreement

    All OUTSIDE groups MUST submit either a Hold Harmless Agreement (see Admin page) or a Certificate of Liability (annual resubmission required, including insurer and policy number. Church ministry groups are exempt from this question.
