Housing in the Residential Hall

It is mandatory to stay in the dorms and to be on the meal plan.

DANCE MASTERS OF AMERICA SHIP ATTENDEE AGREEMENT
In consideration for being accepted into the Dance Masters of America Student Honors Intensive Program, the undersigned, jointly and severally, do hereby release Dance Masters of America, Inc., the Officers of the Organization; the Directors and Assistants of the SHIP Program; the faculty; The Ball State University, from any and all claims for damages, or for injuries which the Attendee may sustain while participating in any and all activities with the Dance Masters of America SHIP program.

*A DOCTORS NOTE stating the attendee is fit to actively participate in the program is REQUIRED WITH THIS APPLICATION.

SPECIAL NOTICE

• This program is for dancers ages 13-18 years of age.
• Late registration will only be accepted if there is space available. Please call Charleen Locasio (504) 454-1376 for availability.
• All Attendees must include a doctor’s letter with the SHIP registration form stating that the attendee is fit to actively participate in the program.
• Attendees should fly into Indianapolis International Airport (IND).

DRESS CODE

• Dancewear should be modest and in good taste.
• Cover-ups should be worn outside the classroom.
• Appropriate shoes for each class. (No socks in place of shoes.)
• Hair must be secured neatly.

• All fees must be paid by the teacher’s check in U.S funds, drawn on a U.S. Bank. 
• Fees paid by check must be mailed to the National Treasurer by the registration date to avoid late fees. (See the first page of the form for deadline dates.)
