Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities organized by the Alliance of Urban United Development and Inclusion (AUUDI) during the selected summer cultural camp program in China.

In exchange for the acceptance of my child’s candidacy by AUUDI, I voluntarily assume all risks and hazards incidental to travel, recreational, cultural, and educational activities, and release, absolve, and hold harmless AUUDI and its officers, staff, volunteers, and representatives from any and all liability for injuries, illness, loss, or damages arising out of travel to, participation in, or return from any activity associated with the program.

In case of injury or incident, I waive all claims against AUUDI and its affiliates, including organizers, group leaders, sponsors, and facility providers. I understand there are inherent risks in travel and group activities, and I accept those risks on behalf of my child.
Medical Release and Authorization

As Parent and/or Guardian of the above-named child, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional of my minor child in the event of a medical emergency, which, in the opinion of the attending medical professional, requires immediate intervention to prevent serious harm, permanent impairment, or severe discomfort.

Permission is hereby granted to the attending physician to proceed with any necessary medical or minor surgical treatment, x-ray examination, or immunizations. In the event of a serious illness, major surgery, or significant injury, I understand that every reasonable effort will be made to contact me prior to treatment. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to AUUDI and its representatives, including Directors, Coordinators, and Chaperones, to provide necessary emergency first aid prior to my child’s admission to a medical facility.

This release is valid throughout the duration of the registered summer program and is executed voluntarily for the purpose of protecting the health and safety of my child while in the care of AUUDI.
