Informed Consent and Acknowledgement

I, the undersigned, acknowledge that I have voluntarily elected to participate in the Summer Cultural Tour in China organized by the Alliance of Urban United Development and Inclusion (AUUDI). I understand that the program involves international travel and includes a range of cultural, recreational, educational, and group activities in various locations across China.

In consideration of my participation, I hereby assume all risks and hazards associated with travel, cultural exchange activities, and any associated transportation, accommodations, and events. I understand the nature of the program and voluntarily release, waive, and discharge AUUDI, its directors, officers, employees, volunteers, and partners from any and all liability for any injury, illness, loss, damage, or other incident arising out of or related to my participation in the program.

I acknowledge that participation in such a program, including international travel, carries inherent risks, including but not limited to accidents, natural disasters, illness, political disruptions, or other unforeseen events.
Medical Release and Authorization

In the event of a medical emergency, I hereby authorize the administration of medical care deemed necessary by a licensed medical professional. This includes, but is not limited to, diagnostic testing, medical or surgical treatment, hospitalization, and the use of medication.

I understand that AUUDI will make reasonable efforts to notify my emergency contact as soon as possible, but I grant permission for emergency medical decisions to be made if I am unable to provide consent at the time of treatment.

I also authorize AUUDI and its designated staff or group leaders to arrange for emergency first aid or medical assistance on my behalf prior to reaching a medical facility if necessary.

I acknowledge that I am responsible for my own health and travel insurance coverage, and I accept responsibility for any medical costs incurred during the program.
