Pricing & Deposits | Submitting this form does not confirm your appointment. A quote will be provided for the piece or session(s). A deposit will finalize your reservation. This non-refundable deposit applies to your balance.

Response time | Please allow up to 72 hours for a response. After submitting, I will contact you as soon as possible with a quote and availability. Consultations are done through messsaging unless otherwise needed/requested. For custom designs, the drawings will be sent to you within 24 hours prior to your appointment (with minor adjustments can be requested if needed).

Location | All tattoo appointments will be at Esthetically Carmen, located at 1553 W. 33rd Street Chicago, Illinois 60608. Appointment-based only.

Tattoo placement(s)

What is your desired placement? Example: bicep, forearm, ankle, etc. Be specific as possible. Placement helps me accurately quote and plan your tattoo.

Tattoo size (in inches only)

“I don’t know” answers are not allowed. If you are unsure, then please take a ruler and measure the area. Example answers: 1x1 or 2.5x2. Size helps me plan the design, pricing, and time required.

If you would like script in your tattoo, please find a font on dafont.com, list the font name & author below, and what exact wording will be.

Be specific with uppercase / lowercase letters for the wording. Type the wording in the “preview box” to see what it looks like in each font. If no scripture, please type in N/A.
