I understand that this booking request form does not guarantee an appointment. I understand that I will be contacted via email within 1-3 days from submitting (if my request has been accepted), and will be required to pay a non-refundable booking/drawing or retainer fee of $60-$160 via Venmo. *the booking/drawing or retainer fee amount is determined by appointment type and total.
