Purpose of this form
1. View and try the following new form options being added to the Payment Request form:
  - Use a unique code to prefill user information.
  - Take a picture of receipts/bills
2. Test your unique code after you have added it with your information.
Unique Code
- This code is defined by you to associate with the following information about you: name, email, phone, address, and ministry team. It can be setup in one of the following ways:
  - Fill out the Add Form Prefill Info form.
  - Send Dianne Richards your requested unuque code and the information listed above.
- The unique code is case sensitive.
- If you haven't added your unique code and information yet and you want to try this form, use my initials: der

- You can add up to 3 photos in this test form.
- Use the rear camera on a phone.
- Use the front photo on a computer or laptop
