    Grooming Request Form
  • Complete a separate form for each dog. This Grooming Request Form applies to all future grooms for this dog. Separate forms are needed so you can note grooming information as well as any behavior or medical information for each dog.

    You do NOT need to submit a new form for each appointment for this dog unless information or instructions have changed. If you ever need to update information or instructions please submit a new form to replace this one.

    If this is your first time with us or if you need to make emergency infomation updates, please complete our Boarding & Grooming Policies & Emergency Information form first. 

    Grooming Services are charged based on the times it takes to complete the service. Your price may go up or down based on how long it takes and can differ from your estimate. If we cannot safely complete the service on your dog, we will stop the service with your dog’s well-being and safety in mind.

  • Will you be reachable during your dog's grooming appointment?*
  • Is this a scheduled appointment, or is your dog staying in boarding with us?*

  • A LA CARTE: If you do NOT want a bath or haircut and ONLY want a la carte services, what would you like? (A la carte services do not typically require bathing first. In order to keep our equipment clean and running safely, any hair that is trimmed on a dog must be bathed prior to the area being trimmed. Foot trimming is a common exception, but may require a foot bath first depending on your dog which increases the price.)
  • FULL BATH/GROOM: If you want a full BATH or GROOM, which would you like? Baths and Grooms always include a nail trim, ear cleaning, teeth brushing, drying, and brushing. Groom includes a haircut. De-shedding is automatically added for dogs who need it but if you know it will be needed please check the box.
  • GROOM: If getting a haircut, what type of haircut would you like for your dog?
  • In ADDITION to a Bath or Groom, I would also like:
  • *Dogs with anal gland issues and ear infections should be monitored by a medical professional. We do not express impacted anal glands or pluck ears with active infections.

  • How does your dog do for grooming?*
  • Does you dog have any special needs, considerations, health conditions, or similar?
  • *If your dog is matted, we will do our best to reach out and discuss the options with you. Small amount of dematting may be an option for dogs who tolerate it well, but large amounts of matting or matting in sensitive areas must be shaved out in order for new, healthy hair to grow back in. Matting is painful and can cause health issues for your dog and can be dangerous to remove and demat. It is always in the best interest of your dog to start fresh with a shorter cut and move forward with a regular grooming schedule (4-8 weeks) to help maintain your dog’s coat and skin. Clipping through matted fur takes great care, meaning extra time. It also wears down our clipper blades quickly. Due to these factors there may be an additional charge for removing extensive matting.
