  • CAMPER REGISTRATION

    AREA 9 & 12 DAY CAMP

    8:30AM-4:30PM

    JUNE 22-26, 2026

    • Full Payment is due at the time of registration!
      • Exception 1: JSLF Scholarship - A minimum deposit of 10% of the total due must be submitted at the time of registration if you have applied for a JSLF Scholarship.
      • Exception 2: Troop Payments - Do not pay anything at the time of registration if the troop is paying full camp fee. 
      • The date of registration will be used as the "Paid in Full" date for these exceptions.
    • Be prepared to provide health information (insurance, immunization/tetanus, history, medications, etc.)
    • Register one camper at a time.
    • Do not use this form for Adult Volunteers or Camp Aides.
    • This form must be completed by a legal parent/guardian.

  • Campers are placed into Program Unit/Level based on their Grade in the Fall of 2026.

    Please contact us if camper will be repeating a grade or has a unique age/grade situation.

  • Pixie or Daisy? 

    Girls entering Kindergarten in the fall might be a Pixie or Daisy.

    • If she is 5 years old by camp AND registered as a GS then she can register as a Daisy.
    • If she is not a registered GS then she must attend camp as a Pixie, and Pixies may only attend camp with an adult volunteer.
  • Cadette+ Unit: This is a regular camper unit. They have the option to stay overnight on Wednesday. This unit also takes an adventurous off-site trip each year which is included in their camp fee.

    Aide-in-Training Unit: This is an instructional training unit for girls who want to be Camp Aides in the future. They are required to stay overnight on Wednesday and Thursday. Each AIT will be evaluated throughout the week on her leadership, teamwork, camp skills, maturity, ability to follow instructions and make sound decisions, and more. AITs who pass the program have the option to be a Camp Aide in the future (or she can return as an AIT year after year).

    Girls who do not wish to go through training, or become a Camp Aide, should register in the Cadette+ unit.

  • STOP!

    This person is old enough to register as a Camp Aide. If she intends to be a Camp Aide - Do not continue with this form - Please complete the Camp Aide/Adult Volunteer Registration Form.

     

    If this person WILL NOT be a Camp Aide, there are 2 options available for them as a camper.

  • Transportation Information

  • Bus Transportation to/from Camp is available for $40.

    Approximate Bus Stop Time: 7:50AM & 5:10PM (Confirmed times will be communicated prior to camp)

    Note: A bus stop must have an adult volunteer riding the bus all five days and a minimum of ten campers to operate.

  • Health History

    This health history is to be completed by a parent/guardian.

  • ALLERGY DETAILS: Please provide nature of allergy and treatment.

    EXAMPLES

    FOOD: Tree Nuts - severe anaphylaxis. Use Epi-pen.

    ANIMAL/INSECT: Cats - causes eyes to water but can be around cats. Treat with benadryl

  • Medications

    • Medications for campers under the age of 18 must be kept  in the health supervisors office (unless otherwise noted).
    • Any medication to be dispensed should be indicated on this form.
    • All medication, including over-the-counter products, should be in the original container and marked clearly with the participants name and directions for use.
    • Prescribed medications and over-the-counter products must be administered in the prescribed dosage by or in the presence of the health supervisor or as per the written instructions other custodial parent, a guardian, or a physician.
    • Medication can only be dispensed to the person named on the prescription container.
    • As per the Safety Activity Checkpoints, medications, including over-the-counter products cannot be dispensed without prior written permission from a girl's custodial parent or guardian.

  • Health Information Privacy Statement From GSUSA

    The health history form is for health care concerns at this camp only. All medical records will be held in limited access by the health care supervisor of this camp. Minimally necessary information may be shared with the camp staff / volunteers in order to provide adequate participant safety and health care. The health history form will be retained by GSK along with all forms / records with noted treatment for 7 years past the age of maturity of the participant. Access to the information will be limited. I have read the above procedures for handling health history information and I agree to the release of any records necessary for treatment, referral, billing or insurance purposes.

  • PHOTO RELEASE AGREEMENT

    Throughout the week, we take photographs and video of the campers to share with camper families. Attendees will receive a link to a private photo album to view photos/videos from that year. We also use a few images and videos on our Facebook account and website. In addition, the council occasionally requests photos from camp to use in promotional materials. Being mindful of the safety of all the children and the privacy of all individuals who participate, we would like to obtain your permission to upload images and videos that may contain your child. Images and videos may be used in the following ways:

    1. Private Photo Album - Area 9 and 12 Google Album 2026 (only viewable by attendees.
      1. If you select "NO" then camper will be removed from the group for group photos and similar.
    2. Area 9 and 12 Day Camp Media and Promotion (Website, Social Media, print materials)
    3. Girl Scouts of Kentuckiana Council Media and Promotion
