I understand that this booking request form does not guarantee an appointment. I understand that I will be contacted via email by Sunday or sooner after submitting this form (if my request has been accepted) and will be required to pay a non-refundable booking/drawing/retainer fee of $80 via Venmo. * The booking/drawing/retainer fee amount is determined by appointment type and total. I understand that there is a $160 appointment price minimum (covers appointments that take 1hr or less); this will be waived for the Mini flash if total is under $160. I acknowledge that until the booking/drawing/retainer fee has been received, my appointment has not been confirmed. I understand that if I request major changes to my design after submitting this form or booking the appointment, the cost may vary from the quote I was initially given or could result in losing my appointment since these requests are accepted based on the original information and details submitted. I have read the FAQ page before requesting to book an appointment. I understand that the artist, Mj Nicole, owns all rights to the design(s) they have created, and I do not have permission to use their designs in any form (e.g., tattoos, prints, logos, merch, flash, publication, etc.). * This field is required.
