  • INTRODUCTION

  • Purpose: The purpose of this survey is for Brown County Public Health to understand how new parents are doing, what challenges they face, what kind of support they need, and what kind of support they get.

Intro: Becoming a parent is an exciting time with many happy moments, but it can also bring stress, worry, and strong emotions. Pregnancy, giving birth, and caring for a newborn can affect anyone’s mental health. For this survey, mental health means how we feel, think, and act, and how we handle stress, connect with others, and make choices. It is important at every stage of life. Maternal mental health focuses on a mother’s mental well-being during pregnancy and up to 18-24 months after giving birth.

Please help by completing this voluntary survey if: You are pregnant, recently had a baby, or were pregnant in the last 2 years and live in Brown County.

Time needed: This survey is about 50 questions and will take about 15-20 minutes to complete.

Thank you gift card: As a thank you, the first 400 people who qualify and give their Brown County mailing address will receive a $20 Amazon, Walmart (Alcohol/Tobacco/Lottery/Firearms Prohibited), or Kwik Trip fuel only gift card. Your name and address will be kept separate from your survey answers. Not sure if you live in Brown County? You can check your address using the county’s online map: https://browncounty.maps.arcgis.com/apps/webappviewer/index.html?id=61fba3fd419045e48aa6ba759838387c.

Voluntary: You do not have to take the survey if you do not want to.

Confidential: Your answers will be kept private. We will only share the results as a summary that will not include your name or address. No one, including Brown County Public Health, will be able to link your answers back to you. Your name and address will only be collected if you are interested in a gift card so we can mail the thank-you gift card. This information will be collected on a different form so your name and address will not be on your survey submission.

  • SECTION 1: ABOUT YOU

We ask these questions to understand who is taking the survey and to make sure we hear from different people in our community. Things like age, ZIP code, race, language, and insurance can affect mental health and the support someone may need. Your answers help us see where services are working and where more help is needed.

  • SECTION 2: YOUR PREGNANCY/BIRTH/POSTPARTUM EXPERIENCE

We ask about your pregnancy, birth, and postpartum experiences because these events can affect mental health. Some questions may feel difficult, but your answers will help us better understand the challenges parents are facing in our community.

People experience trauma in different ways, even people who go through the same situation often react differently. Some people are unsure whether their experience counts as trauma. Something does not have to be extreme or life- threatening to be traumatic. What matters is how the experience felt to you. If you felt scared, unsafe, helpless, or like things were out of your control during your birth experience, it may have been traumatic for you.

Some people may not realize their experience was traumatic.

    Examples can include:

    • Unexpected medical procedures (such as an emergency C-section)
    • Severe pain that felt overwhelming
    • Being separated from your baby after birth
    • Problems or complications during birth
    • Feeling rushed or pressured into decisions
    • Feeling like your concerns were not listened to
    • A past trauma coming back during labor or birth
    • Fear for your life or your baby’s life

If you had a difficult birth experience that still affects how you feel, think, or respond when you think about it, it may have been difficult or traumatic for you.

  • PLEASE READ BEFORE NEXT QUESTION

Some people may have experienced harm or unsafe treatment from a partner or family member during pregnancy or after having a baby. Domestic abuse is an ongoing pattern of behaviors that one intimate partner uses to control the other. It may include manipulation, threats, name calling, isolation, intimidation, stalking, blame, and/or physical harm.

These experiences can affect mental health in different ways, such as stress, anxiety, sadness, fear, or feeling overwhelmed. Everyone experiences this differently.

You do not need to share anything you are not comfortable sharing. You may skip this question if you choose.

If this brings up difficult feelings or concerns about your safety, support is available:
