Please read the following information carefully before beginning your application:

This form is intended for organizations. If you are applying as individual artists and collectives., please complete the Artist/Collective Application Form instead. We strongly recommend that you draft your responses in a separate document and save your work before copying your answers into this form. This will help prevent any loss of information in the event of a technical issue.

Applicants must clearly indicate their total requested amount. The maximum request is $10,000.We encourage all applicants to attend one of our upcoming Information Sessions, which will include a Q&A and guidance on completing the application.
