My name is Marty, and I’m a disabled, immunocompromised and covid-conscious queer person living in Washington State, USA.

I put together this survey to gather submissions for a “Community Care Catalogue”: an up-to-date listing of Covid-conscious (CC) small businesses, organizations, products, services, event organizers, and mutual aid asks. Who can add submissions to the catalogue?

Covid-conscious people around the globe who want to advertise their:
- Small business selling products or services
- Community group that serves the CC community
- Hosts of ongoing / multiple events specifically for the CC community (both virtual and in-person masks-required, such as theater, exercise classes, community events, etc). Due to the broad scope of this catalogue, I'm not adding individual events at this time.
- Add mutual aid requests for free here.
What do I need to know about this survey?

The information you provide will be used for the catalogue! It will take about 10-15 minutes to complete. I'll clearly mark whether questions are required, optional, or not used directly in the catalogue. I am only able to provide a catalogue in English at this time, but encourage others to reach out if they want to start one themselves in another language!

What’s the cost?
- Sliding scale from $0 - $150 for those who want to advertise in the catalogue - pay what you can. Receipt provided for tax purposes. I also welcome bartering! Email me at ccCommunityCare@proton.me if you'd like to contribute non-financially.
- FREE for those to view and shop from the catalogue! (if you just want to view it, sign up here to get notified when new additions are added)

By participating in this survey, you consent to the information you share being compiled into a “Community Care Catalogue,” which will be public. You can remove your responses at any time by emailing me at ccCommunityCare@proton.me and I’ll remove your information from the digital catalog.

Examples: 1. Our shop has masked Mondays where all staﬀ and customers wear masks. We are wheelchair accessible with a ramp at the door and in-store bathroom with a wide door , grab bars, and accessible sink. Changing table provided in bathroom. Our event space upstairs is not wheelchair accessible. 2. Fully virtual support group over Zoom with auto-captions and optional participation via speaking, writing, or video. Any final comments, questions or feedback? This is my first attempt at this catalogue, and feedback and ideas are very welcome. I am disabled so I may take some time to respond to questions.

I (Marty) am a disabled, trans, immunocompromised community member who's taken on this project both as a way to support our community and to financially support myself. My hope is that it will bring value to your small business and organizations. Since it's brand new, I don't know how many people it will reach, but I anticipate at least a few thousand.

If you have the funds, please contribute via my linktree, which has a sliding scale from $5 - $150.

You will receive a receipt which you can use for tax purposes. I am not a nonprofit so it cannot be written off, but it can be reported as a business expense for advertising. It takes a lot of work to create a project like this, and I am low-income myself, and invite those with the resources to contribute generously if they are able. You'll get a receipt for tax purposes to report as a business expense.

If you are financially struggling like me, do not feel pressured to contribute - consider this my gift to you. I also welcome bartering if you'd like to contribute in a non-financial way, but you don't have to.
