Step 2 - Summer Program Payment.  You may pay for your child  / children's summer program fees one of the follwoing ways:

Emergency Information

Summer Program Rules Of Conduct 

This is a 6-week program run by parent co-op.  It is a morning program consisting of swimming, tennis and canoeing. Lots of additional activities can be part of the program based on parent volunteers such as sailing, golf, soccer, arts and crafts and evening activities. There are even activities for the parents including adult tennis and exercise classes!

A few important facts to note:

1. This is NOT a drop off camp. All children must be supervised by a parent.  If you are unable to be present for one of the days you must arrange with another parent at the community centre to supervise your child

2. If you wish to go for a run/walk/swim/paddle at the LMCC while your child is participating in the program you must inform your child who the point of contact is while you are engaged in your activity

3. Children are not allowed to swim unless they are supervised by an adult

4. Tennis shoes must be worn on courts at all times

5. Dogs are not permitted on the premises

6. There are two fees to participate in the program. The first fee is to join the Community Centre and the second fee is the weekly program cost. Please refer to the LMCC newsletter for fee structure

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by the Lake Manitou Summer Program during the selected camp. In exchange for the acceptance of said child’s candidacy by the Summer Program, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless the Summer Program, the LMCC, and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected activities.

In case of injury to said child, I hereby waive all claims against the Lake Manitou Summer Program and the LMCC, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including basketball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

Medical Release and Authorization

As Parent and/or Guardian of the named children, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to the Lake Manitou Summer Program, the LMCC, and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered season.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

RELEASE: I hereby release CCLM and the Lake Manitou Summer Program from any and all claims and liability of any kind of personal injury or property damage while participating in the Lake Manitou Summer Program. By participating in the summer program, I understand that there are inherent risks associated with the practice of such activities. I certify that my child(ren) is in good health. I agree to inform the summer program of any medical conditions. If attention is required for illness or injury, I give permission to a staff member for such care.

BY CLICKING YES, THAT WILL HAVE THE SAME EFFECT AS AN OFFICIAL SIGNATURE.  THIS CONFIRMATION WILL BE EQUALLY BINDING AS AN ORIGINAL SIGNATURE.
