• POCT License Eligibility Screening

  • Are you employed by a laboratory in WV which holds a nonwaived CLIA certificate?*
  • Do you have at least a high school diploma or equivalent?*
  • Have you ever held a WV laboratory practitioner license?*
  • Some employers handled all aspects of licensing on behalf of employees and, consequently, sometimes individuals do not remember ever having a license. Please perform a search* at https://services.wvlablicense.wv.gov/verify/ and change your response to Yes or No to continue. 

    *Be sure to search using all former names you may have had. If your last name has ever changed, we recommend searching just your first name so you don't have to perform multiple searches.

    If you have an existing license that is eligible for renewal and submit an application anyway, your application will be denied and payment will not be refunded. 

  • What is the status of your license?*
  • Your expired license is eligible for renewal and you do not qualify for a new license. You must login to your LAB-PLUS account to renew. If you have not registered for an account, please submit a request at https://form.jotform.com/253514221356046.

  • You must login to your LAB-PLUS account to renew.* If you have not yet registered for an account, please submit a code request at https://form.jotform.com/253514221356046.

    *You can register at any time, but you will not be able to access the renewal form unless your license expires within 60 days. If you register prior to the renewal window, you will receive an email notice when it opens. You may use the optional CE reporting form within your account to report CE obtained throughout the year. 
  • Are you a licensed respiratory therapist?*
  • Do you know what tests you will be performing?*
  • The following tests are exempt from the POCT licensing requirement when performed by a respiratory therapist licensed to practice in WV:
    • ABG/VBG
    • Oximetry
    • Potassium
    • Chloride
    • Sodium
    • Ionized calcium
    • Glucose
    • Lactate/lactic acid
  • Will you be performing other tests NOT listed in the paragraph?*
  • You are exempt from the licensing requirements (effective July 1, 2026) and our office does not require you to have a license. However, your employer has the right to require a license regardless of the available exemptions. Please verify with your supervisor whether a license is required before submitting your application, as refunds will not be issued for applications that are submitted in error or unnecessarily. 

    If you will perform any other nonwaived tests not listed herein now or in the future, you must obtain a POCT license prior to testing.

  • Please print or screenshot the list of exempt tests and ask your supervisor to compare it to the tests you will perform before applying. If you will be performing only the tests in the list, you are exempt (effective July 1, 2026). However, your employer has the right to require a license regardless of the available exemptions. Refunds will not be issued for applications that are submitted in error or unnecessarily.
