  • This form must be completed by a parent or adult swimmer.

    The enrolment end date must be at least 30 days from the date this form is submitted.

     

    Please consider if one of the following options would be helpful before completing this form.


    Place training on hold
    A request can be made to hold a swimmer's place for up to 8 weeks. To arrange this, submit a Squad Hold Request.


    Reduce training     
    Training can be temporarily reduced to 1 class per week for up to 3 monhs. To arrange this, submit a Reduced Training Request.
