2026-2027 Cub Scout Pack 3017 New Member Enrollment

IMPORTANT: This enrollment form is intended for NEW scouts only.  If you are re-enrolling your scout in Pack 3017, please use this form.

• Please submit a separate form for each Cub Scout who is joining Pack 3017 for the 2026-2027 year.
• After submitting the form and the pack dues payment (payment instructions in section 7), your scout’s pack enrollment will be complete.
• Please note: Submitting this form does not guarantee enrollment in the Pack and within a Den. Den formation depends on meeting size limits and adult volunteer requirements, which is why your participation as a parent-volunteer is so vital!  
• Once we confirm your spot, you will need to register online with Scouting America, our national organization.  There, you will need to create a scout account and pay an additional annual fee. DO NOT PAY THE SCOUTING AMERICA FEE UNTIL YOU HAVE CONFIRMATION FROM THE LOCAL PACK 3017 REGISTRAR (email: pack3017registrar@gmail.com).  We will provide instructions when you need to complete this action.

If you have another family member who you would like to include on scout communications, please add their name, email and phone number.

Measure a shirt that fits your child and match it to the size chart. Sample shirts are also available upon request.

NOTE: Shirt sizing runs small and your child will use this shirt for a couple of years. We recommend ordering a size or two up.

If you need a larger size than what is offered, please click other and someone will contact you.

Sizing Tips & Special Orders:

• Order Up: Sizing runs small; if you are between sizes, we recommend ordering a size larger.
• Custom Sizes: If you require a size not listed in the charts, select "Other" and we will contact you to assist.

Adults Only:

• If you need more than one shirt and they are different sizes you may select more than one box
• If you want two shirts of the same size, please indicate that selection in the "Other" option.
• If you already submitted a response for this question while completing the form for another Cub Scout in your family, please select 'None Needed' so as not to order the shirt twice.

• Local Pack Annual Dues are $250 for all ranks. (Please note that there is an additional fee of ~ $135 USD for annual national dues that will be paid through Scouting America at another time).
• To make payment for the Local Pack Annual Dues, we prefer you pay by PayNow to our treasurer Dawn Kaung at +65 9059 4815.
• If you need to pay cash, please reach out to Dawn Kaung at +65 9059 4815 to arrange.
• All dues must be received by 31 August in order to process your enrollment into Pack 3017.

To keep your Scout ready for adventure, please submit the Annual Health and Medical Record to Pack leadership by September 15th. This essential form provides leaders with critical health history and grants legal authorization for emergency medical treatment if a parent cannot be reached. Because it must be updated annually to remain valid, please ensure all sections are complete and that your Scout’s tetanus immunization has been received within the last 10 years.

I, the parent/guardian of the above Cub Scout, do hereby give permission for my child to participate in all regular den meetings, pack meetings and special events of Cub Scout Pack 3017. I know of no physical or psychological impairment that could prevent his/her participation in any activity in the Scouting America program for his/her age group. Furthermore, I hereby agree to hold harmless all adult leaders of Pack 3017 (individually or collectively) and Scouting America from any liability in the event of accidental injury or sudden illness to my child whilst engaged in, or on the way to and from, such activities. In the event of injury or sudden illness occurring to my child whilst under the supervision of adult leaders of Cub Scout Pack 3017, if urgent medical attention is required, I hereby give my permission for said adult leaders of Cub Scout Pack 3017 to attend to that treatment according to their best judgement.
