This form is to be completed by meet directors no later than 2 weeks prior of your meet. If you submit within 2 weeks of your event, and rush shipping is needed, you will be charged the rush shipping fees for your packet.

Starting3/14/2026, we will be using new (Orange) Chain of Custody drug test forms. With these forms, you will need to use the Athlete Notification Form.

Starting 5/9/2025, we will no longer be sending out the drug test instructions with your meet packets. If you need a copy of the instructions for your competition they can be found and printed at the link below.

This doesn't have to be an exact number. We just want to make sure you are shipped enough supplies. For example if you submitted a packet for 43 lifters and then allowed 2 more to make 45, you don't need to update the form because for both it would only require 3 tests. If you went from 60 lifters to 61 though that would require one more test. So, if you want to submit your form early so you don't forget and don't plan on going over what you originally sanctioned for, you can request this a little early, but don't submit if it may be cancelled.
