      1. Fill out the form by selecting the desired model along with Accessories. All accessories are priced separately from the device. Confirm the correct Connector Type is selected for your chosen device..

      2. An optional 7 Year Extended Limited Warranty can also be selected, the cost based upon the model selected. Only valid for 110/120V systems used in the USA

      3. Agree to the Extended Limited Warranty, but only if you have selected it.

      4. Enter your name, email, phone and address.

      5. PROMOCODES:

      Should you have a PROMOCODE enter it. The final price will be adjusted in accordance to the discount percentage assigned to the Promocode.

      For order shipped outside the USA, enter INTSHIP as the PROMOCODE. The shipping fee will be $00.00.   The buyer must arrange for international shipping, typically throguh DHL International.  You will find the discount either next to the selected product or under the shipping fee on this form. Our office will assist you in this process.

      8. Once Submitted you will receive a pop-up "Thank you". If you selected "ACH",  we will then contact you to verify your submission and review any questions or considerations you may have.

      9.  In any case, once all is verified, a formal invoice will be generated and sent for your signature and your order will be processed. 


      * Be advised that devices do not automatically carry a 7 year Extended Limited Warranty. However, we have provided an option to choose one, if an Extended Limited Warranty is important to you.

              Extended Warranties / Training
              Extended Warranties - Available for US and Canadian purchases only . See pricing per model In addition to the Standard Warranty, an Extended Limited Warranty, when purchased, is handled as a maximum out-of-pocket expense of $750.00 in any given year for maintenance or repairs for parts only. This means any repairs, after the original Standard Warranty period expires, will never cost more than $750.00 in any given year during the Extended Limited Warranty period. Be advised that an Extended Limited Warranty does not cover the cost of periodic maintenance for PEMF Devices fabricated with a spark gap chamber. Only the PEMF Systems, Inc repair facility can fix or replace a spark gap chamber. An Extended Limited Warranty does not cover labor, delivery loops, Mag Discs, power cords or any accessories. An Extended Limited Warranty included with the purchase of a new device will NOT be transferred to a new owner if the original buyer sells the device.

              3 year Factory Warranties for new devices can be transferred to a new owner for a one-time $25.00 administrative fee.
