  • Please fill out all required fields to request the reservation of equipment belonging to Academic and Institutional Technology. Equipment reservations should be submitted at least one business day before pickup. A confirmation email will be sent when the reservation has been approved and processed.

    This form is only to be used by Faculty/Staff or Students who are part of a student club, organization, or ResLife and have been pre-approved to checkout special equipment.
