  • How to have your child Baptized at Saint Thomas the Apostle

    Important: All documents must be received thirty (30) days before baptism.

    A. Register as a parishioner: REGISTER HERE

    Note: You must be a registered parishioner at least

    4 months before baptism. 

    B. Attend a Baptism Preparation Class (Session) 

    C. Provide copies of your documents 

    1. Child's Birth Certificate - you cannot submit this form without attaching the child's birth certificate as a .pdf to this form. 
    2. Parent's Sacramental Certificates (Baptism, Eucharist, Confirmation, Matrimony)
    3. Sponsor's Sacramental Certificates (Baptism, Eucharist, Confirmation, Matrimony)
    4. Sponsor's Letter of Good Standing (if from another parish)
    5. Sponsor's Letter of Preparation (if from another parish)
  • If you will be a Sponsor (Godparent), please fill out the Sponsor Form.


  • How to have your child baptized at another Catholic church:

    If you are a parishioner of St. Thomas the Apostle, and desire to have your child baptized at another parish, then you will need a Letter of Good Standing and/or a Letter of Permission from the Pastor.

    If the parish where your child is to be baptized is within the Diocese of Fort Worth, you must provide to that parish:

    • Verification of all your sacraments received (Baptism, Confirmation, Eucharist, Marriage)
    • Verify that you are a registered member of Saint Thomas
    • To obtain a letter of Good Standing we need:
      • Full Name and Address of the Church
      • Full Name of the Pastor of the Church
      • Full Name of the celebrant of the sacrament(s), if different from the pastor.
      • Full Name of the Child, and the Child’s parents.
    • Note, requesting permission does not guarantee that our Pastor will grant it, nor does it guarantee the Pastor of the church in question will permit it.

    If the parish where your child is to be baptized is outside the Diocese of Fort Worth then:

    • It is your (not the parish’s) responsibility to receive permission from both the Diocese and parish where the sacrament of baptism will take place, and permission from the Diocese of Fort Worth and the Pastor of St. Thomas the Apostle.
    • Note, requesting permission does not guarantee that it will be granted.

  • Requirements for Parents

    1. At least one (1) parent must be a Catholic in good standing.
    2. Both parents must be willing to raise the Child as a Catholic.
    3. Must submit any sacraments with sacramental certificates (Baptism, Confirmation, Eucharist)
    4. If married in the Catholic Church, verify the sacrament with a Church Matrimony Certificate.
    5. Provide a copy of the Child's Birth Certificate.

    Note: For all baptisms being celebrated by the clergy (a Priest or a Deacon) please submit #1 a copy of the birth certificate and #2 all sacramental records as .pdf files only via email to baptisms@sttafw.org

  • Requirements for Sponsors (Godparents):

    1. Must attend baptism preparation sessions, either at Saint Thomas or their own parish.
    2. Must be 16 years or older, in good standing with the Church, and a proper example of Christian life.
    3. Must be Sacramentally Initiated (Baptism, Confirmation, Eucharist)
    4. If married, must be in a valid, Catholic marriage.
    5. Must verify their sacraments by submitting sacramental certificates.
    6. If from another parish, must verify their good standing with a letter, signed by the pastor.
    7. If there are two sponsors, one must be male and one must be female.
    8. Parents may not be sponsors.
    9. Non-Catholics may not be sponsors, but may witness the Baptism.
