Thank you for your interest in booking. Please kindly fill out this form for any custom tattoo requests! After submission, expect an EMAIL from me (miki.s.tattooart@gmail.com) within 3 days. And please check the Spam or Junk mail folder just in case it went there :)

If your request is accepted, I will send a quote or ask any additional questions needed to bring your idea to life!

I charge by piece, and current minimum charge is $150 + 5% gst. I will send you quote base on the tattoo idea and the detail info you submitted.

Please note I do not tattoo these areas: finger, ear, toes, feet, palm, sole, scalp, genital areas. I also advise against finger tattoos as they may heal unevenly and can require multiple touch ups throughout the years - I will tattoo the area if you are aware and okay with the risks and cost for touchup. *Placement affect the appointment time and cost of the tattoo.

Please include a picture of yourself with the desired placement from a good distance. Draw an outline on the image and show the size. *

Detail description of your tattoo idea. (Eg. What you want exactly in your tattoo? Shaded OR only lines? Color OR black and grey? Is there scar/ existing tattoo for cover-up/ elevated mole/ birth mark/ any skin condition? Additional note on size/ placement?) *I cannot work on keloids scar.*

*We do not copy other artist’s work or give same copy of other client’s tattoo design. If the style fits, we can do similar but not exact.

Reference pictures. (Please attach pictures for tattoo placement, and for design inspiration, eg. style of drawing you are looking for. Rough draft of your idea would help a lot for explaining and understanding your idea too. )*

I will reply back to all booking requests, however please understand I may decline requests that I cannot do/ not interested in. If your booking request is accepted, please expect the following: (you must check all boxes below to be considered.)*

After we have agreed on booking date and design, a 24-hour hold will be provided on the agreed date and time slot. A $50-$150 non-refundable deposit (depending on size and complexity of the design) will be required to confirm your appointment time. If the deposit is not received within 24 hours, the hold will come off.Design will be provided around 1 week before your appointment. You’re allowed 2 free minor revisions, and each additional revision will be with $25 fee. Major revisions or complete redrawing (eg. original request is a rose and want to change to sunflower) a $50 re-design fee will be required, or it could be rejected. **No changing on design within 72 hours before appointment.Tattoo Size: Actual tattoo must be within 1 inch difference from the original request. (as it would affect the appointment time we need).Reschedules are allowed (only ONCE) with at least 72 hours notice before the appointment. Otherwise the appointment will be considered a cancellation.I reserve the right to refuse service to anyone who arrives to the appointment intoxicated or under the influence of drugs and alcohol.Please arrive no later than 10minutes after your appointment time, otherwise you may be considered cancellation.
