P.ink Artist/Shop Interest Form

This form is for artists who would like to get involved with Personal Ink, either by:

• Applying to be added to our directory for decorative mastectomy and/or scar tattoing and/or aerola tattooing, and be automatically eligible (but not required) to participate in our P.ink Angel Fund
• Hosting or participating in a private P.ink Day event at your shop

Personal Ink is launching a new initiative, the P.ink Angel Fund, where we will be sponsoring decorative tattoos for port scars, lumpectomy scars, and male mastectomy scars. The mastectomy sessions generally last 5-6 hours, and lumpectomy and port scar sessions take less time. We will reimburse you up to $1200, depending on the final design. Payment will be provided upon completion.

If you would like to be considered for this program, you must be able to schedule an appointment with the applicant within 3 months of being contacted and be able to execute a tattoo in the style that the applicant desires.

All artists who are added to our directory are automatically eligible for this program. You have the option to opt out in the artist application.

How it Works

1. If a P.ink Angel Fund candidate requests you on their application, we will first reach out to you to make sure that you are interested, able to execute a tattoo in their desired style (we will provide the candidate’s application form with that information and reference materials), and able to schedule a session within the next 3 months.
2. We will do our best to contact you. However, if we don’t hear back from you within 2 weeks of initial contact, we will need to move on to the next identified artist to ensure timely access for recipients.
3. P.ink will facilitate initial communication between you and the recipient and get the session scheduled. As part of these communications, we’ll ask about any specific shop rules around guests, etc.
4. Once a design is agreed upon, we will need you to submit an initial invoice with an estimate of the cost, as well as complete an Artist Agreement and a W9 form. Links will be provided to you. Upon completion of the tattoo, you will need to submit an updated final invoice along with photos/videos of the finished piece. Reimbursement will be provided only upon receipt of the final invoice and photos/videos.

 

 

 

 

 

 

Every October, tattoo studios across the country close their doors for a day to create free, curated healing experiences for recipients who have had mastectomies and are seeking closure through coverage of their scars with beautiful artwork. All tattoos are donated by talented and trained tattoo artists who are experienced in tattooing over scar tissue successfully. All events are organized and facilitated by trained Personal Ink volunteers, and recipients are screened via our annual application process.

At these events, recipients receive decorative mastectomy tattoos on their surgical site. These tattoos are designed to be completed in one day.

Please keep in mind that we have a roadmap for expansion of our P.ink Days. It may be some time before we are able to onboard your shop, if that is what you're interested in. We will happily reach out to discuss options with you.
