This form is for Girl Scouts in the GSNYPENN Pathways council serving parts of New York State and Pennsylvania. If this is not your council, do not submit this form. Contact your own council for instructions.
This form is to be used after completing your pre-approved* service hours.
*If you did not request pre-approval for your service, please do so now. Then, wait until you have pre-approval before submitting this form. Please note that requesting retroactive approval does not guarantee it will be granted.
Instructions:
- Make sure you have the correct form. This is for the Service to Girl Scouting Bar. (The Community Service Bar final summary form is here.)
- Submit this form for completed service to Girl Scouts within one year of receiving council pre-approval.
- Attach your verified log of service.
- Allow up to 10 business days for council to review your final summary.
- Once your final summary is approved, you and your troop leader/adult volunteer will receive a congratulatory email.
- After you receive notification that you have earned the award, the Service to Girl Scouts pin may be purchased from our council retail store. Our staff can confirm your award status.
