CMT Report Form

This Report Form is completed by any Member of the Commissioned Ministry Team (CMT) to document a Meeting with a Commissioned Minister that has active Standing in the Mid-America Region. This Report Form creates a thorough record of the formal conversation between the Commissioned Minister and their CMT. This completed Report Form will be stored in the Commissioned Minister's clergy file for future reference.

Mid-America's Regional Requirements for all Commissioned Ministers

Commissioned Ministers are required to complete the following prior to each Meeting with their CMT for Recommissioning.

The Application for Renewal of Commissioned Ministry [Appendix 8].
Certificate Program Coursework (if applicable).
15 hours of Continuing Education annually. (Please document and submit this information using the Continuing Education Report Form.)
Annual Standing Renewal Form (to maintain Standing with the Mid-America Region).
Renew both Boundaries Training (BT) & Anti-Racism/Pro-Reconciliation (ARPR) Training every 3 years. (Please refer to the Mid-America Regional Website for more information and to register for upcoming Trainings at https://www.mid-americadisciples.org/upcoming-events.)
Attend Clergy Cluster Meetings. (Please contact your Ministry Associate in the Springfield Regional Office directly at sgf@ccmadisciples.org for more information about, and to be added to, a Clergy Cluster Email Group.)
