  • How this form works

  • The Owner/Pilot, Authorised WOF Inspector, and Admin will automatically receive a PDF copy of their application.

    Each step of the signature process will be actioned automatically, when the required email fields are filled, once the applicant submits the form.

    This form is a controlled document, managed by the NZHGPA, under the delegation of the Civil Aviation Authority (CAA).

     

  • Form Instructions

  • 1. Complete all areas where applicable;

    2. Harnesses and emergency parachutes must pass this inspection every 12 months, or every 100 hrs, or at the manufacturer's recommended interval, whichever comes first;

    3. Student and Tandem passenger harnesses, karabiners, and back protection must meet specific NZHGPA requirements;

    4. There are two options for completing this form:

      i. Owner/Pilot Checks: checks on recreational glider equipment may be carried out by the OWNER/PILOT, with minimum PG2 certificate, under supervision of an Authorised WOF Inspector.

      ii. Authorised WOF Inspector Checks: checks can be done soley by the Authorised WOF Inspector;

    5. If you are an Authorised WOF inspector assessing your own glider equipment, select Authorised WOF inspector.

    6. If equipment does NOT pass inspection, fill out OPMF10_Equipment Grounding Notification and drop file in relevant section.

    If the Authorised WOF Inspector has any doubts as to the airworthiness of this equipment, they shall refer to the equipment manufacturer and NOT sign this form;

    The Authorised WOF Inspector accepts NO RESPONSIBILITY for defects caused after further use of the equipment.
