I understand that this booking request form does not guarantee an appointment. I understand that I will be contacted via email within 1-3 days from submitting (if my request has been accepted), and will be required to pay a non-refundable booking/drawing or retainer fee of $60-$160 via Venmo. *the booking/drawing or retainer fee amount is determined by appointment type and total. I understand that there is a $140 appointment price minimum (will be waived during certain sales). I acknowledge that until the booking/drawing or retainer fee has been received my appointment has not been confirmed. I understand that if I request major changes to my design after submitting this request and/or booking the appointment, the cost may vary from the quote I was initially given or could result in losing my appointment since these request are accepted based on the original information and details submitted. I have read the F.A.Q page before requesting to book an appointment. I understand that the artist, Mj Nicole, owns all rights to design(s) they have created, and I do not have permission to use their designs in any form (ex. tattoos, prints, logos, merch, flash, publication, etc.). *
