I understand that this booking request form does not guarantee an appointment. I understand that I will be contacted via email in 1-7 days after submitting this form (if my request has been accepted) and will be required to pay a non-refundable booking/drawing/retainer fee of $60-$180 via Square. * The booking/drawing/retainer fee amount is determined by appointment type and total. I understand that there is a $160 price minimum that covers tattoos that take 45 minutes or less. I acknowledge that until the booking/drawing/retainer fee has been received, my appointment has not been confirmed. I understand that if I request major changes to my design after submitting this form or booking the appointment, the cost may vary from the quote I was initially given. I have read the FAQ page before requesting to book an appointment. I understand that the artist, Mj Nicole, owns all rights to the design(s) they have created, and I do not have permission to use their designs in any form (e.g., tattoos, prints, logos, merch, flash, publication, etc.).
