High Complexity License Eligibility Screening

*Be sure to search using all former names you may have had. If your last name has ever changed, we recommend searching just your first name so you don't have to perform multiple searches.

Note: effective July 1, 2026, MLT and MLS license types were combined into a new category known as High Complexity Testing Personnel (HCTP). If you ever held an MLT or MLS license, it was automatically converted to an HCTP license. Please keep this in mind and answer the following questions based on having an HCTP license. If you did not have an MLT or MLS license, please disregard this message. 

If you have an active MLT license and are attempting to obtain an MLS license, you do not need to take further action since the categories were combined. If you have already registered for an account, you can login to update your certification information, if necessary. If you have not yet registered for an account, a letter with a one-time registration code will be mailed approximately 2 months prior to expiration. You can request a code at any time by completing the code request form at https://form.jotform.com/253514221356046. 

If you have an MLT license that expired fewer than 5 years ago and are attempting to obtain an MLS/new HCTP license, you are not eligible to apply. Please register for an account if you do not already have one and submit a renewal application. If you do not have a letter with your one-time code, please submit a request at https://form.jotform.com/253514221356046. 

If you have an MLT or MLS license that expired more than 5 years ago, you are eligible to apply for a new license.

You must login to your LAB-PLUS account to renew.* If you have not yet registered for an account, please submit a code request at https://form.jotform.com/253514221356046.

*You can register at any time, but you will not be able to access the renewal form unless your license expires within 60 days. If you register prior to the renewal window, you will receive an email notice when it opens. You may use the optional CE reporting form within your account to report CE obtained throughout the year.

Please note:

• Education documentation (degree listing concentration or transcript) is required, which must show your name and the school's name.
• If you are certified, you are also required to provide evidence of certification, which must show your name, the issuing entity, and the certification title. Information for obtaining appropriate documentation from ASCP and AMT can be found here.
• We suggest uploading your education and certification documents within the application instead of selecting the email option for faster processing, so please have them ready before you begin the application.
• If you are applying under a route that requires previous experience and it was not gained under a WV trainee license, you must submit evidence by email to dholsclp@wv.gov after you apply. Information regarding acceptable documentation can be found here.
