  • Have you ever held a WV laboratory practitioner license in any category?*

  • Please perform a search* at https://services.wvlablicense.wv.gov/verify/ and change your response to Yes or No to continue. 

    *Be sure to search using all former names you may have had. If your last name has ever changed, we recommend searching just your first name so you don't have to perform multiple searches.

  • Are you applying for the same license category? For example, previously had an MLT license and are now applying for an MLS license.*
  • What is the status of your license?*

  • Your expired license is eligible for renewal and you do not qualify for a new license. You must login to your LAB-PLUS account to renew. If you have not registered for an account, please submit a request at https://form.jotform.com/253514221356046.

    *You can register at any time, but you will not be able to access the renewal form unless your license expires within 60 days. If you register prior to the renewal window, you will receive an email notice when it opens. You may use the optional CE reporting form within your account to report CE obtained throughout the year.
