    If you want to submit an event but don't need to request funding, this is the incorrect form. Please exit this form and fill out this one ATTACHED

  • Format: (000) 000-0000.
  • Is this request intending to purchase equipment for your organization rather than a specific event?*
  • If you plan on bringing an external group to campus, you will automatically recieve a follow-up email to fill out the form titled, 'Student Org External Event Request'.

  • Is this event a fundraiser? Will there be a fundraising portion?*
  • Does this event involve the purchase, sale, or consumption of alcohol?*
  • If your event involves alcohol, you will need to submit a form that will be sent to you following submission of this form. If the event is happening off campus, you will be asked to provide the establishments liquor license and their proof of insurance. You also have the option to meet with risk management if you prefer.

  • This next section will help you request funding to help your event happen! Here are some things to know! 

    There are some items that cannot be funded, these include the following: For more information, view the Registered Student Organization Handbook. 

    • Personal Items or Services 
    • Alcohol, firearms, or ammunition, or drugs
    • Telephone
    • Animals 
    • Radioactive or hazardous materials 
    • Wagers or gambling 

    1) Once you submit your funding request, you will be required to attend a Student Government Association Meeting *they will outreach to you

    2) Following this meeting, you will either be approved, or denied with feedback for resubmission. 

    3) Once approved, you will recieve a check that you will deposit into your organizations bank account. Don't have a bank account for your organization? Reach out to the Hub or visit them in Cisler Hall! 

    *Checks are cut each Wednesday, please remember this is not an instantaneous process. You will be notified via email from housing@lssu.edu that your check has arrived. Ex; If you are approved for funding on a Monday, the Wednesday that's 9 days later is when you'd ideally get your check if there are no hiccups.

  • Below, you will have the option to select the categories your organization is requesting funding for, as well as the dollar amount requested. You will also asked to provide an explanation of funding. This needs to be detailed. If it is not detailed, it will be sent back for resubmission. If your organization will be traveling, additional questions will populate.

  • Are you intending to use a Motor Pool LSSU vehicle for your travels?
  • Please know that if you are going to be using motor pool for your request, the cost will be automatically subtracted from the funding you recieve. Prior to submitting this form, you will be sent a form to submit a motor pool request. You will also be sent a form that will have you input the students who will be traveling with you for this event. We recognize that attendance may not be solidified by the time you're filling out this form. Please provide the roster of attendees 24 hours in advance of travel via the form. If you already know, you can fill it out now. 

  • Before recieving the funding if approved, your organization MUST have a bank account. Funds must be directly deposited into this account. 

     

    In order for the business office to issue the check, you need to provide your EIN number, and a completed W-9. If you have not submitted them, you will recieve a form after completion of this form collecting this information.

     

    This is a non-negotible part of the process for reciving funding. If we do not recieve this information in a timely manner, you will not recieve funding in a timely manner.
