As a Candidate for Ordained Ministry through the Seminary or Apprentice Track, you will work directly with your Regional Minister to form an Ordination Nurture Team (ONT). Your ONT is a special group of individuals that will help you explore and develop your skills and gifts for Ministry. Your ONT will offer ongoing support, guidance, and encouragement throughout your Ordination journey.

This form allows you to provide the Regional Commission on the Order of Ministry (RCOM) with the Names and contact information for all Members of your ONT.

*** Important Information about ONT Meetings ***

As a Candidate for Ordination, you will particpate in three separate Meetings with your Mentor, your Congregational Leader/Elder(s), and all Members of your ONT collectively on a monthly or bimonthly basis. Each Meeting will be documented using a Meeting Report Form. All Meeting Report Forms are stored in your clergy file to be read and reviewed by Members of the Regional Commission on the Order of Ministry (RCOM).

For each ONT Meeting, any Member of your ONT will submit a Report using the ONT Report Form [Appendix 7].
For each Mentor Meeting, your Mentor will submit a Report using the Mentor Report Form [Appendix 8].
For each Congregational Leader/Elder Meeting, your Congregational Leader/Elder will submit a Report using the Congregational Leader/Elder Report Form [Appendix 9].

I understand that I can submit a NEW Appendix 5 anytime should a Member of my ONT change.
