You will work directly with your Regional Minister to form your Ordination Nurture Team (ONT). Your ONT is a special group of individuals that will help you explore and develop your skills and gifts in ministry. Your ONT will offer ongoing support, guidance, and encouragement throughout your Ordination journey. Your ONT will also hold you accountable to the Ordination process.

This form allows you to provide the names and contact information for all Members of your ONT.

*** Important Information about Meetings with ONT Members ***

You will have separate Meetings with your Mentor, your Congregational Leader/Elder(s), and all Members of your ONT collectively. Please note that Meetings with your Congregational Leader/Elder(s) can include meeting with either/or of your Sponsoring and/or Supporting Congregational Representative(s). Your separate ONT, Mentor, and Congregational Leader/Elder Meetings can take place in person, over the phone, or through Zoom or other similiar video chat service. The Regional Commisison on the Order of Ministry (RCOM) asks that you meet monthly, or at least every other month, with your ONT, Mentor, and Congregational Leader/Elder.

A Meeting Report Form will be used to document each Meeting you have with your ONT, Mentor, and Congregational Leader/Elder. All Meeting Report Forms are submitted directly to your Ministry Associate in the Springfield Regional Office to be stored in your clergy file; and are reviewed by all RCOM Members prior to each of your upcoming RCOM Interviews.

For each ONT Meeting, any Member of your ONT will submit a Report using the ONT Report Form [Appendix 7].
For each Mentor Meeting, your Mentor will submit a Report using the Mentor Report Form [Appendix 8].
For each Congregational Leader/Elder Meeting, your Congregational Leader/Elder will submit a Report using the Congregational Leader/Elder Report Form [Appendix 9].

I understand that I can submit a NEW Appendix 5 anytime should a Member of my ONT change.
